1. What do I need to bring?

Please bring the following to your appointment:
- Completed diet questionnaire
- Proof of household income for all people living in your house
- Proof of identity
- Proof of identity for child
- Proof of residency

2. What are the eligibility requirements?

Eligible participants include:
- Breastfeeding women until the infant’s first birthday
- Children up to the age of 5
- Infants
- People with a nutrition risk factor
- Postpartum women for 6 months
- Pregnant women
- Residents of Kansas within income guidelines
